Social Service Manager-Skilled Nursing Facility

Responsibilities

Provide support and advocacy for resudents and their families. Coordinate psychosocial assessments and individualized care plans. Assist with discharge planning, referrals, and community resources. Participate in interdisciplinary care plan meetings. Promote resident rights, dignity, and quality of life. Maintain accurate documentation and suport regulatory compliance.

Qualifications

Bachelor's degree required in Social Work, Human Services, Psychology, Sociology, or a related field. Experience in long-term care or skilled nursing facility required. Strong communication, organizational, and interpersonal skills. Knowledge of resident rights and discharge planning. Social Woeker license in North Carolina a plue (LBSW, LSW, LCSW, LMSW, etc.)
