Relief/Per Diem Case Worker

Position: Relief/Per Diem Case Worker Locations: Bedford/Concord/Action areas Salary: $21.22 per hour Schedule: Per Diem/Relief: Days (10a-6p) and Weekends (9a-3p). 20 Hours of Availability Preferred About Us: Walnut Street Center is a nonprofit human service agency that provides support services to adults with Intellectual Disabilities, assisting them in living engaged and meaningful lives: opportunities to live, work, volunteer, and socialize within the community.

Who we are looking for: Seeking per diem case workers to help empower adults with intellectual disabilities to make more meaningful life choices by working directly with those individuals in our Independent Living Supports (ILS) Department Work Settings: Individual's Homes/In the Community The Job Responsibilities: Ensuring community engagement Assisting with ensuring a livable environment Participating in the development, implementation, evaluation, and documentation of each individual's goals Providing therapeutic support and assisting in Behavior Support Plans, if appropriate Transporting individuals to and from appointments Assisting individuals with Activities of Daily Living such as feeding, dressing, toileting, and other aspects of personal care, and nutritional needs Money management, such as support with benefits Willingness to obtain training & certification within 90 days of hire

Job Requirements: High School Diploma or equivalent Valid MA driver’s license and willingness to drive personal vehicle Ability to pass a driving and background check COVID-19 Vaccine Card Ability to communicate effectively in English Ability to complete training both online and in-person Preferred: Previous human services or long-term care experience is strongly preferred CNA/HHA experience preferred Associate’s or bachelor’s degree in a related field preferred Weekdays 10:00am-6:00pm or Weekends 9:00am-3:00pm
