Follow the patient Plan of Care (POC) as directed by RN Case Manager and PCP Communicate necessary revisions to patient status and/or needs to RN Case Manager and Clinical Manager when applicable Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
