RN Transfer Center Coordinator - Care Integration - Full Time - Days

Reporting to the RN Clinical Manager of Command Center Operations Center, Manager of Emergency Services, the RN Transfer Center Coordinator is responsible and accountable for the act of coordinating both internal and external patient care needs by facilitating patient placement in the appropriate internal nursing unit or external hospital transfer utilizing pre-established criteria and consulting with members of the inter-professional team.

Adheres to National Patient Safety Goals as appropriate based on level of patient contact. With hundreds of requests each year, the RN is the link to patient-centered care using teamwork, inter-disciplinary communication and collaboration to provide dynamic environment for ongoing development of the movement and placement of patients.
