Job description
The Licensed Clinical Social Worker (LCSW) and Licensed Clinical Professional Counselor (LCPC) position assesses and treats individuals with mental, emotional or substance abuse issues in a school or the main River. Stone Health Clinic. Duties include individual and/or group therapy, crisis intervention, case management, client advocacy, prevention and education.
The position consults with physicians, school staff, and other clinic staff to provide solution-focused care. The position coordinates and connects patients and clients to appropriate social services to meet the physical, financial, social and/or environmental needs of patients and their families as needed. Work as a member of a care team within a Patient Centered Medical Home model of care
