Job Summary

We are seeking an Occupational Therapist (OT) to provide high-quality therapy services to clients in their homes. The ideal candidate will have experience working with elderly and/or disabled patients who require physical therapy to improve mobility, balance, and functionality. The Physical Therapist will assess patient conditions, develop personalized treatment plans, and provide therapeutic interventions to achieve optimal outcomes.

Responsibilities

Conduct initial & ongoing patient assessments to determine diagnosis and treatment plans Develop and implement treatment plans to improve mobility, strength, endurance, and pain management Provide therapeutic treatments and procedures, including therapeutic exercise, neuromuscular reeducation, and manual therapy Counsel patients on their condition and rehabilitation goals Advise patients on pain management and self-help therapies Update patient health records with treatment plans, discharge instructions, and notes on patient condition Maintain professional licensure with state authorities Collaborate with other health professionals to achieve optimal patient outcomes Medical Specialties: Geriatrics Home Health Physical & Rehabilitation Medicine Join our team and enjoy a supportive work environment, opportunities for growth, and the chance to make a meaningful impact on the lives of your patients.

Qualifications
