You’ll work closely with patients, caregivers, and their care teams to improve independence, functional abilities, safety, and quality of life. Your responsibilities

Providing comprehensive occupational therapy evaluations Developing individualized treatment plans based on each patient’s needs Providing therapeutic activities and interventions to improve functional independence Training patients in activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assessing patients’ home environments and recommending appropriate safety modifications and adaptive equipment Providing education and training to patients and caregivers Monitoring patient progress and modifying treatment plans as needed Collaborating with physicians, physical therapists, speech therapists, nurses, and other members of the care team Completing accurate and timely electronic documentation Home care offers a unique opportunity to get to know your patients and make a meaningful difference in their daily lives.

Requirements
