Job Title: RNAC MDS Coordinator Location: Randallstown, Maryland Company: Patapsco Healthcare About Us: Patapsco Healthcare offers post-hospital, short-stay nursing and rehabilitation in private and semi-private rooms, and long-term care services for residents who call us home. Our skilled nursing facility is unique in that we have on-site hemodialysis unit, which allows patients and residents to receive dialysis treatments without leaving the facility.

We also have a secured memory support unit for residents with Alzheimer's or dementia. Our interdisciplinary clinical care team includes a full-time nurse practitioner and full-time physician. Patapsco Healthcare is a We Honor Veterans partner.

Job Summary

Patapsco Healthcare is seeking a dedicated and compassionate RNAC MDS Coordinator to join our skilled nursing facility team. As a vital member of our care team, you will collect and assess information for the health and well-being of our residents, collaborating with staff, residents, and their families to ensure that individual care plans reflect the type of care required.

Responsibilities

Collect and assess information for the health and well-being of residents from admission to discharge Plan, document, and submit compiled information for resident care plans Develop and maintain individual care plans that reflect the type of care required Maintain complete healthcare records and reports, including PCC being utilized according to best practices Investigate patient/family complaints and take appropriate actions to bring to resolution Collaborate with staff, residents, and families to ensure that individual care plans reflect the type of care required Requirements: Current Maryland Registered Nursing Licensure (or compact/multi-state license) BLS/CPR certification In-depth knowledge of MDS, nursing care methods, health, and safety guidelines MDS certification by the American Association of Nurse Assessment Coordinators (AANAC)-required RN required with experience Preferred Skills: Experience with Point, Click, Care platform, Management experience Benefits: $5,000 Sign on Bonus Referral Bonuses Premium Holiday Pay Daily.
