Licensed Practical Nurse

The Licensed Practical Nurse (LPN) provides direct nursing care and clinical support to residents in a skilled nursing facility under the supervision of a Registered Nurse and in collaboration with the interdisciplinary care team. This role is responsible for administering medications and treatments, monitoring resident conditions, documenting care, communicating with providers and families, and supporting resident quality of life through person-centered care practices.

The LPN also provides guidance and delegation to Certified Nursing Assistants (CNAs) and assists in maintaining a safe, therapeutic, and homelike environment for residents.
