Therapeutic Foster Care Case Worker - Gustavus Adolphus

Therapeutic Foster Care (TFC) Case Worker Sagewell Family Services at Gustavus Adolphus Position: Full-time, 40 hours per week; non-exempt Compensation: $22.00–$27.82 per hour, commensurate with education and experience Schedule: Monday through Friday, with hours varying according to case needs; participation in a rotating on-call schedule for 24/7 crisis response is required Reports to: Foster Care Supervisor Make a Difference for Children and Families Sagewell Family Services is seeking a compassionate, dedicated professional to join our Therapeutic Foster Care team.

This position offers meaningful opportunities to apply case management, advocacy, and family engagement skills while supporting youth in foster care and the families who care for them. The Case Worker coordinates placements and services, develops individualized treatment plans, and provides ongoing guidance to youth and foster families.

Working collaboratively with community providers and an interdisciplinary team, this professional helps promote placement stability, strengthen family connections, and support each youth’s well-being.

Key Responsibilities

Coordinate intake, screening, and placement of youth referred to the program Develop and update treatment plans in collaboration with youth, foster families, and the interdisciplinary team Maintain accurate, timely case records, including Family Assessment and Service Plans (FASPs), court reports, and progress notes Provide ongoing support through scheduled visits, family engagement, and crisis intervention Connect youth and families with community resources and advocate for needed services Participate in team meetings and coordinate care with service providers Provide foster parent training, support, and supervision Assist with program outreach and relationship-building with referral sources Participate in the on-call rotation to support crisis response Follow agency policies, safety procedures, and applicable state requirements, and complete required in-service training Adapt to evolving program needs and related responsibilities Qualifications: A bachelor’s degree is required; a master’s degree is preferred.

Any exception to the degree requirements requires an approved waiver from the New York State Office of Children and Family Services (OCFS). Access to personal transportation, a valid driver’s license, and vehicle insurance meeting New York State requirements.
