Referral Coordinator

As the Referral Coordinator for Swope Health Services, you will be a part of a team of caregivers that provide care to the people who need it the most. The Referral Coordinator processes and tracks referrals to outside PCMH department The PCMH emphasizes a team-based approach to health care delivery. This is accomplished by delivering proactive, planned and coordinated evidence-based care.

The Referral Coordinator will accomplish this by their primary function of processing and tracking of referrals to outside ancillary services.
