Position Summary
The Pharmacy Assistant I is responsible for assisting the organization, pharmacists, care managers, primary care providers and external partners in the management of the member’s needs related to medications.
Essential Functions
Work to support CCNC Pharmacist and Care Management teams in the coordination, implementation, and ongoing management of pharmacy-related initiatives. Support and assist Pharmacy and Care Management teams with the medication management process by collecting and accurately transcribing medication lists from pharmacy claims data or pharmacies, primary care practice records, discharge summaries, and other sources, as requested, into the CCNC Care Management documentation platform, in preparation for medication reconciliation/ medication review.
Access Hospital/Data or Electronic Medical Record system, as appropriate. Respond to Help Desk inquiries regarding prescription claims, benefit coverage, and prior authorization processes. Document interactions and escalate complex issues to senior pharmacy staff or clinical teams as needed. Support Pharmacists with data gathering for disease state management panels Engage with community pharmacies and external partners in an effective, professional manner.
Adhere to CCNC privacy and security policies to ensure that patient and company data are properly safeguarded.
Qualifications
Completion of Pharmacy Technician Program, recommended. Certified Pharmacy Technician (CPhT) and registered with NC Board of Pharmacy, must obtain within 6 months if not already certified and registered. A minimum of 2 years of relevant pharmacy experience is required. Ability to gain and maintain access to Hospital/Data or Electronic Medical Record systems is required.
Knowledge, Skills, and Abilities Critical thinking skills, independent decision making, and problem-solving abilities. Strong knowledge of CCNC Care Management documentation platform Computer skills required, including the ability to work with various software applications (MS Office preferred) Ability to review multiple data sources and accurately transcribe requested information.
Experience with the appropriate use and navigation of clinical/claims data systems Knowledge of and experience with federal and state regulations governing prescription medications and pharmacy operations Excellent interpersonal communication skills Sensitivity to diversity of cultures, language barriers, health literacy and educational levels Responds to change with a positive attitude and a willingness to learn new ways to accomplish work activities and objectives Able to shift strategy or approach in response to the demands of a situation Working Conditions Routinely, there may be some minor physical inconveniences or discomforts in the work setting, including sitting for moderate periods of time.
Must be able to utilize office equipment, computer, keyboard, and phone with or without assistive devices. Repetitive wrist motion and occasional lifting/carrying of up to 25 pounds. Multiple contacts are required with various members, providers, multi-payer systems and community partners to ensure coordination of services.
Travel may be required within the region and/or state.
