Home Health Clinical Team Manager (RN)

Opportunities for personal and professional growth. Position Details: Location: Troy office, with a hybrid schedule. Hours: Monday to Friday, 8:30 am to 5:00 pm.

Additional Requirements: Weekend backup and weekly rotation of evening on-call duties (calls only).

Responsibilities

Receive and review case referrals. Assess patient needs and assign appropriate clinicians. Provide guidance and support to ensure quality home care services. Collaborate with the team to establish therapeutic goals and develop comprehensive care plans. Conduct quarterly record reviews and make recommendations for improvement.

Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.

Qualifications

Registered Nurse with current licensure in Michigan. Minimum 2 years of management or supervisory experience in healthcare, preferably in home care. Current CPR certification.
