Case Manager
Job description
Description PURPOSE: Provides support to the participants of the Cardinal Manning Center (CMC).
Key RESPONSIBILITIES
General Tasks:
- Participate in problem solving, staff and program development with the guidance of supervisor.
- Provide intense case management services to assigned clients.
- Develop effective, trusting relationships with clients with a focus on facilitating their move towards independence.
- Provide crises intervention, short-term and long-term case management services.
- Housing search and placement for clients including accompanying clients to housing appointments if needed.
- Provide information, referrals, and social services advocacy.
- Maintain accurate and timely documentation of client information.
- Network with agency and outside service providers, ensuring client access to appropriate services.
- In concert with supervisors assume responsibility for attending assigned community meetings. Meetings will be assigned based on the needs of the department.
- Support to weekly skid row seminar series.
- Participation in supervision, case management, and staff meetings.
- Community outreach and collaboration with other social service agencies.
- Other duties as assigned. WORKING CONDITIONS & PHYSICAL WORK DEMANDS:
- Requires ability to sit at computer for long periods of time and move about on foot.
- Stoop, kneel, bend at the waist and reach on a daily basis.
- Requires standing and some travel time. Varied shifts including evening and weekend may be required.
Requirements
Bachelor’s degree in any human service-related field; experience with Microsoft office; experience with homeless population. Bilingual Spanish/English preferred. ESSENTIAL JOB COMPETENCIES:
