Sud Case Manager

Provide case management for an active caseload of twenty-five to thirty youth/wards and interfacing between Juvenile Probation Department and the Community Adolescent Treatment Programs. The Case Manager is responsible for assessment, intervention plan development, and referral services. Conduct observed collection of urine samples for testing, follow-up and monitor youth/wards cases while enrolled in SUDS.

Conduct ongoing home, school and treatment program visits with youth/ward, family, treatment providers and school teachers. Participate in the team meetings, compile and report monthly progress reports to Probation. Maintain case files on all participants. Enter data in the PCMS database and provide end of the month stats to the data entry/admin staff.
