Job description
Patient Care Coordinator Reports to: Outpatient Services Supervisor Location: on-site Status / Schedule: full time hours 9a-5p FLSA Status: Non-Exempt About the Role Under general direction, the Patient Care Coordinator runs the day-to-day front desk operations of our Rehabilitation Center and helps ensure every patient has a positive experience.
You will be the first person patients and visitors meet, and you will keep scheduling, registration, insurance verification, and authorizations moving so patients can get the therapy they need.
What You'll Do
Greet patients and visitors in the Rehabilitation waiting room Answer phones, respond to inquiries, and direct callers to the right personnel Schedule Physical Therapy (PT) and Occupational Therapy (OT) appointments, including patients from the "Ready to Schedule" work queue Remind patients of appointments and notify them of changes by phone or letter Register patients in the system and arrive them on the day of their appointment Obtain required signatures for processes and records Verify insurance eligibility, including new-month eligibility on outstanding referrals Collect co-pay and deductible payments Confirm valid authorization is in place for outside PT patients referred for PT or OT, and enter treatment authorizations into the system before the date of service Notify primary care physician(s) when authorizations have expired Request extensions for internal therapy patients Identify "no show" patients and reschedule them as directed by PT/OT Keep your manager informed of delayed cases Fax forms and documents as needed Follow federal and local laws on patient privacy Perform other duties as assigned What Success Looks Like Approved authorizations entered into the system within two (2) business days Positive patient relations, reflected in patient feedback including complaints and compliments Strong planning and prioritization of workload and assignments Responsiveness to shifting department priorities Qualifications Required High school diploma or GED 1–3 years of related experience Ability to comply with HIPAA guidelines Good listening skills and the ability to give full attention to patients and medical professionals alike Ability to perform highly detailed work with sustained attention while handling numerous inquiries Preferred Experience in a healthcare environment Basic understanding of medical terminology and procedures Bilingual in Spanish Physical Requirements and Work Environment Regularly required to stand, sit, use hands to finger, handle, or feel, reach with hands and arms, and talk or hear.
Frequently required to walk. Occasionally required to climb, balance, stoop, kneel, crouch, or crawl. Regularly lifts and/or moves up to 10 pounds. Vision requirements include close vision, distance vision, color vision, depth perception, and the ability to adjust focus. The work environment can be busy, noisy, and interruption-filled, and you may interact with individuals who are dissatisfied.
