Patient Care Manager (Hospice)

The Patient Care Manager is responsible for coordinating and managing a team of patients. The Patient Care Manager plans, organizes, and directs all patient care services for patients in the assigned caseload ensuring quality patient care is maintained. He/she coordinates with the inter-disciplinary team to implement the established plan of patient care and ensure effective manage-ment of visit utilization and control of expenses and supervision of patient care personnel.
