DEPARTMENT: Wyndstone POSITION TITLE: Wynd. Stone LPN RESPONSIBLE TO: Administrator EXPOSURE LEVEL: Category III EXEMPTION STATUS: Non-exempt ______________________________________________________________________________________ SUMMARY: Works under the general supervision of the Administrator and Health Director to provide quality professional, nursing care to Wynd.

Stone residents. The Wynd. Stone nurse works cooperatively and respectfully with residents, coworkers, families, medical staff, visitors, and volunteers to create a positive and safe environment. ESSENTIAL JOB FUNCTION: Nursing

  • Provides nursing care to residents of Wyndstone by assessing Elders and overseeing the administration of nursing treatments. Works closely with providers to determine appropriate care.
  • Promote and ensure professional standards of nursing care and elder satisfaction with both elder and family involvement; observes elder status as necessary to assure quality care and coordinates with the rest of the Wyndstone care team to assure continuity of care.
  • Identify changes in condition; provide assessment, identify needs, communicate with providers, and initiate/direct treatments or interventions.
  • Responds to and leads emergency situations
  • Performs record keeping according to policies developed by St. John’s United in accordance with state and federal regulations. Accurate documentation includes, but is not limited to: incident reports, medication errors, nursing notes, scheduled assessments & vitals.
  • Develop and maintain Care Plans and participate in family meetings as well as regular communication with families on resident status and plan of care.
  • Assist with Admit/Discharge process and required documentation, including communication with the case management team.

  • Attend and contribute to team meetings
  • Ensures communication and collaboration between various shifts to ensure nursing treatments are being completed accurately and timely.
  • Works with the Administrator and Health Director in developing and implementing measures to improve the quality of elder care and improve processes
  • Other duties as assigned. Follows policy and procedures on annual influenza vaccinations. Attends all mandatory in-services REQUIREMENTS: Minimum Requirements: Must be licensed as a Licensed Practical Nurse in the State of Montana. Previous work experience in a health care setting preferred. Knowledge, skills, and abilities required:
  • Knowledge and skills acquired through completion of a state certified Licensed Practical Nurse or Registered Nurse Program
  • Skills in observation of human behaviors or changes of condition
  • Ability to understand and express written and verbal communication. Ability to interact effectively and professionally with Elders, family members, coworkers and other clinical support staff

  • Ability to understand and to contribute to Elder charts and care plans
  • Ability to relate to and work with the ill, elderly, disabled, emotionally upset, and at times, combative people within the community
  • Ability to respond to Elder needs in a timely and professional manner, recognizing the Elder’s rights
  • Ability to deal with stressful situations (i.e. Death and Dying) PHYSICAL DEMANDS:
  • Requires lifting, bending, walking, and standing for prolonged periods of time. Must be able to lift 25 pounds on a regular basis and up to 50 pounds occasionally. WORKING CONDITIONS:  Work involves some exposure to hazards, bloodborne pathogens, or physical risks, which require following basic safety precautions.  Normal health care environment.  Work involves exposure to plants and animals within the environment.
