Job Summary

As the team Vocational Specialist, this member of the ACT multidisciplinary team has training in rehabilitation/counseling , and clinical skill experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients with severe and persistent mental illnesses under the clinical supervision of the team leader and the psychiatrist.

Essential Functions

Predominantly responsible for all vocational, educational related duties such as assessments, motivational Support and vocational counseling. Provide Job Coaching and development. Assess and evaluate client's skills, abilities and capabilities for potential educational / vocational opportunities Complete Vocational Assessment.

Coordinate community linkage and referrals for vocational opportunities. Assess and monitor client's treatment progress for job readiness and placement. Address symptoms of mental illness that interfere with work Provide case management and coordinate services with Employers, Social Security Department and Social Services to assist client in their effort to maintain employment Assist client with life and social skills as well as community linkage.

Provide support and advocacy around housing, money management, Wellness management in their effort to find and maintain competitive employment Required Background and Experience: At-least one year experience in job search, employment counseling or vocational rehabilitation. Bachelor's degree in Social work, human services or related field required.

Masters Degree preferred Experience working with adult population with severe chronic mental health condition, substance abuse, and medical conditions. Knowledge of vocational and educational assessment , of care plan, and timely documentation. Knowledge of best practices and evidence of base care management Knowledge of community resources and advocacy.
