Our mission is to help patients regain independence and comfort in their own homes. We’re seeking dedicated Physical Therapists (PTs) to join our growing team for part-time or per diem visits. • Evaluate patients’ physical conditions, limitations, and rehabilitation needs
• Develop and implement individualized treatment plans
• Provide therapy in accordance with the physician’s Plan of Care
• Supervise and coordinate care with PTAs and HHAs
• Maintain accurate documentation and progress notes
• Educate patients and caregivers on exercises and equipment use
• Assess home environments and recommend modifications
• Collaborate with nurses, social workers, and other healthcare providers
• Submit visit notes on time while maintaining HIPAA compliance Qualifications: 🎓 Qualifications:
• Licensed Physical Therapist (PT)
• Minimum 1 year of PT experience preferred
• OASIS and Well. Sky (Kinser) experience preferred (training provided)
• Current CPR certification
• Physical exam (within last 12 months)
• TB test (within last 12 months)
• Valid driver’s license
