Key Responsibilities

Clinical Assessments: Perform comprehensive nutritional assessments and risk screenings for residents upon admission and routinely throughout their stay. Personalized Care Plans: Develop, monitor, and adjust individualized nutrition interventions, therapeutic diets, tube feedings, and wound-healing meal plans. Interdisciplinary Collaboration: Work closely with Nursing, Speech Therapy, Physicians, and Culinary Services to ensure holistic resident care and attendance at care plan meetings.

Quality & Compliance: Maintain full compliance with state/federal healthcare regulations and sanitation guidelines, participating in facility quality assurance (QAPI) initiatives. Resident Connection: Educate residents and families on therapeutic diets and gather preferences to enhance dining satisfaction.

Qualifications

1+ years of clinical nutrition experience in long-term care, post-acute rehab, or hospital setting preferred. Two-year associate degree program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND).
