You do NOT have to cover all of Montgomery County.

What You'll Do

Complete physical therapy evaluations in patients' homes Develop individualized plans of care Help patients improve strength, balance, endurance, and mobility Provide therapeutic exercise and functional training Provide gait, transfer, and stair training Assess fall risks and home safety Help patients regain mobility following illness, surgery, injury, or hospitalization Educate patients, families, and caregivers Monitor patient progress and modify treatment plans Complete reassessments and discharge planning Collaborate with the interdisciplinary care team Complete timely clinical documentation using Well.

Sky What We're Looking For Active Maryland Physical Therapist License Required Graduate of an accredited Physical Therapy program Reliable transportation Strong clinical and communication skills Ability to work independently in patients' homes Home Health Experience Preferred, but Not Required Well. Sky Experience is a Plus Less Windshield Time.
