Clinical Associate I, II - Intensive Case Management

The Southcentral Foundation (SCF) Clinical Associate is a member of a multi-disciplinary care team who facilitates skill development and provides direct care in the community, residential, or outpatient clinical settings for customer-owners. The Clinical Associate facilitates highly interactive and participatory care, while supporting customer-owners to build knowledge and navigate problems with a focus on positive outcomes.

Clinical Associates will participate in the coordination of customer-owner care, support, access to community resources, case load management, and group facilitation as appropriate for program.
