SOCIAL WORKER-Weekends

Seeking a Weekend licensed social worker. The Hospice Social Worker provides compassionate, supportive services to hospice patients, families, and caregivers in the community. This position works in the field and plays an important role in helping patients and families navigate the emotional, social, and practical challenges associated with end-of-life care. The Social Worker collaborates with the interdisciplinary hospice team to assess patient and family needs, develop and implement individualized plans of care, and connect patients and families with appropriate hospice and community resources.
