You’ll collaborate with patients and their care teams to ensure care needs are addressed while building relationships that make each visit meaningful. Your responsibilities

Performing wound care and other skilled nursing interventions Administering medications and assisting with medication management Providing catheter care Helping patients manage diabetes through ongoing care and education Evaluating general wellness and reporting changes in patient condition Educating patients on disease management and recommended care practices Coordinating follow-up after medical appointments Assessing supply needs and arranging appropriate medical supplies Maintaining complete and timely electronic patient records Providing care in the home allows you to focus on the individual behind the diagnosis.

Requirements
