MDS Coordinator Registered Nurse Oshkosh WI

Are you an experienced RN with a passion for coordination, quality care, and leadership? Evergreen is looking for for a dynamic Medicare Coordinator � Registered Nurse to join our Health Center team! In this exciting role, you�ll do more than just complete MDS paperwork � you�ll be a key resource, a team leader, and an advocate for excellence in resident care.

What You�ll Do: Coordinate and manage the Medicare program for the Health Center. Complete accurate and timely Minimum Data Set (MDS) documentation. Serve as a knowledgeable resource to unit staff, promoting best practices and collaboration. Step in as a staff nurse when needed to ensure seamless care delivery. Assist in oversight of day-to-day operations and ensure high-quality service for residents in Creekview Rehab.

Participate in the on-call rotation, offering support, consultation, and leadership coverage. Be an active member of the Creekview Leadership Team, contributing to strategic planning and operational excellence. What We�re Looking For: A licensed Registered Nurse (RN) with a strong clinical background. Experience with MDS, care planning, and Medicare coordination is required.

This position is 32 hours per week. The work hours are Monday, Wednesday-Friday 9am-4pm. Tuesday 11am-6pm. On call coverage is every other weekend from 4pm on Friday through 6am on Monday.
