Case Manager/Care Coordinator Level 2 – School-Based Services Southern Highlands Community Mental Health Center Wyoming County, WV | Full-Time Southern Highlands Community Mental Health Center is seeking a full-time Case Manager/Care Coordinator Level 2 to join our School-Based Services team in Wyoming County. This position works with children, adolescents, and their families to help connect them with essential behavioral, medical, social, educational, and health-related supports.

The Case Manager/Care Coordinator works closely with families, school personnel, and service providers to coordinate care, advocate for students, and support the development and implementation of person- and family-centered treatment plans.

Key Responsibilities

Assist children, adolescents, and families in accessing needed behavioral, medical, social, educational, and health-related services and supports. Coordinate care with families, school personnel, providers, and other involved supports. Advocate for the needs of students and their families. Assist with the development and implementation of person- and family-centered treatment plans.

Help connect students and families with appropriate resources and services.

Education

& Experience Requirements: Applicants must meet one of the following qualifications: Bachelor’s or Master’s degree in a human services-related field, per West Virginia Board of Social Work requirements; OR Licensed Psychologist with a Master’s or Doctoral degree from an accredited program; OR Licensed Registered Nurse.
