• GREELEY, CO, Salary Range: $28.00 - $34.00Hourly, The Center at Centerplace is recruiting for a Case Manager to assist with patient placement referrals, assessments, care plans, maintaining patient records, audits and other duties as assigned. Essential Responsibilities Complete psychosocial assessments and maintain resident social histories. Coordinate discharge planning, including communication with hospitals, families, physicians, insurance providers, and community agencies. Assist residents and families with discharge needs, including home health, hospice, durable medical equipment, transportation, and community resources. Provide education regarding Medicare, Medicaid, insurance benefits, advance directives, and available financial assistance programs. Participate in interdisciplinary care conferences, resident care planning, and MDS meetings. Advocate for resident rights and assist in resolving resident and family concerns. Coordinate referrals to community resources and support services. Maintain accurate and timely documentation in Point. Click. Care and resident medical records. Collaborate with nursing, therapy, admissions, dietary, activities, and other departments to ensure continuity of care. Monitor discharge goals and barriers, proactively working to facilitate safe and timely transitions. Assist with regulatory compliance, survey readiness, and quality improvement initiatives. Maintain knowledge of federal, state, and facility regulations related to resident care and discharge planning. Perform other duties as assigned.
