What You'll Do

Complete physical therapy evaluations in patients' homes Develop individualized plans of care Provide therapeutic exercise, gait, balance, transfer, and mobility training Help patients improve strength, endurance, mobility, and independence Assess home safety and identify fall risks Develop and reinforce home exercise programs Educate patients, families, and caregivers Monitor progress and adjust treatment plans as needed Complete reassessments and discharge planning Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary team Complete accurate and timely documentation using WellSky What We're Looking For Active Washington, DC Physical Therapist License Required Graduate of an accredited Physical Therapy program Reliable transportation and ability to travel between patient homes Strong clinical and communication skills Excellent time-management and organizational skills Ability to work independently in patients' homes Home Health Experience Preferred, but Not Required WellSky Experience is a Plus Why Human Touch?
