JOB TITLE: SFP Prescription Refill Nurse DEPARTMENT: Seneca Family Practice (SFP) JOB RELATIONSHIP: Responsible to: Clinic Manager/NVCH Director of Nursing Responsible for: No other employees Interrelationships: Works cooperatively with all hospital departments, medical staff, outside resources, patients, and visitors.

JOB SUMMARY

Provide any necessary information, assistance, orders or instruction to patients, hospital departments, pharmacies, outside medical offices or any other facility as required by staff provider or supervisor as well as perform necessary patient assessments and enter documentation in current record system in accordance with state licensing requirements and SFP Operating or Nursing Policies & Procedures.

Primarily responsible for refilling prescription medications as needed by patients per physician order/refill policy. This nurse is also responsible for checking outdates on medications kept at SFP, as well as ordering medications as needed from the hospital pharmacy. This position will provide quality improvement activities as necessary and or as directed.

These activities may include data analysis, data collection, and quality improvement. JOB QUALIFICATIONS: Experience: Previous clinic office experience preferred but not required.

Education

Associates or Bachelor Degree with LPN or RN from an accredited institution. Certification: Current Kansas LPN or RN license required with IV certification preferred for LPN applicants. Current BLS/CPR certificate required. Job Duties: (This is not an all inclusive list) Refill patient medications as instructed by providers and written protocol.

Perform triage patient assessments for service determination. Enter orders into EMR as instructed by providers. Maintain/order medication and samples inventory. Address patient questions via phone or face-to-face contact. Administer injections as instructed by providers. Complete ancillary testing as instructed by providers.

Check mediation outdates on a monthly basis. Check inventory of medications and restock as needed. Provide backup for SFP Clinic nurses and aides as necessary.
