Transition Support Coordinator

As a Transition Support Coordinator, you will be responsible for providing face-to-face and/or virtual Specialized Transition Services to members seeking services within community settings. You will work collaboratively with Care Managers to facilitate member transition and community living, housing navigation, resources, and funding options.

You may also complete review of person-centered care plans regarding services needed based on set guidelines. The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living.

Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
