Health Coach

This role involves conducting comprehensive care assessments and engage residents in health goal-setting. Implement lifestyle interventions to reduce health risks, support chronic disease self-management, and improve quality of life through sustainable behaviour change. Collaborate with the Community Health Team to ensure holistic care planning and effective service delivery.

Job Responsibilities

Assess residents' health status and risk factors through subjective and objective evaluation Facilitate collaborative goal-setting with residents and stakeholders using S.M.A.R.T. framework Design tailored health coaching plans to guide residents towards wellness goals Support residents in addressing lifestyle and behavioural change challenges Track and document resident progress and health outcomes Job Requirements Diploma/Advanced Diploma in healthcare-related field Ability to work independently and as part of a team Strong communication and social engagement skills, particularly with older adults Excellent organisational, coordination, and interpersonal abilities Proficiency in computer skills and Microsoft Office applications
