LTC Community Health Worker

The Long-Term Care Community Health Worker (LTC CHW) supports residents in a five-unit, 24/7 residential group home serving individuals with developmental disabilities across a range of ages and support needs. This role promotes wellness, independence, and quality of life through person-centered care, assisting with activities of daily living (ADLs), daily routines, and meaningful engagement.
