Responsibilities

Working with Kendal~Crosslands Communities primary physicians within a Collaborative Practice Agreement to provide medical on-site care and services for residents in our community. Identifying and managing chronic and acute health conditions by requesting laboratory and/or x-ray studies, initiating treatment and/or managing medication regimens with appropriate follow-up.

Responding to emergencies and administering first aid to residents by evaluating injuries and arranging further treatment if necessary. Participating in planning for residents’ needs as they transition through levels of care. Collaborating with Director of Resident Care on any present or anticipated problems. Performing medical procedures as necessary.

Adhering to Pharmacy protocols and procedures including e-prescribing. Administering treatments, injections and medications to residents as prescribed/necessary. Communicating lab and test results to residents and physicians. Managing INR Lab results and follow up as necessary. Providing wellness screenings. Initiating requests for consultations or referrals.

Participating in Community Quality Assurance, Pharmacy and Infection Control Committees as requested by Director. Arranging and establishing orders for transfer to the Health Center or hospital and assisting with discharges from hospital back to Independent Living and or to Skilled Nursing.
