Registered Nurse Case Manager (Home Health)

Join Our Team! We are currently seeking a compassionate and dedicated Registered Nurse Case Manager to join our team at Home Nursing With Heart in Omaha, NE. As a member of our team, you will have the opportunity to provide patient-centered care to individuals in the comfort of their own homes.

Responsibilities

Conduct initial assessments and develop individualized care plans for each patient Coordinate and provide skilled nursing care to patients in their homes Collaborate with other healthcare professionals to ensure the highest level of care Educate patients and their families on their conditions and treatment plans Provide emotional support and guidance to patients and their families Qualifications: Valid RN license in the state of Nebraska Prior experience in home health or case management Strong communication and organizational skills Ability to work independently and as part of a team Commitment to patient-centered care If you are passionate about providing patient-centered care and making a difference in the lives of others, we would love to hear from you.
