Guide patients in their health journey through behavioural change and motivation Bridge care gaps between healthcare providers and community resources Create meaningful connections with patients to support their health goal Drive better health outcomes through integrated care coordination Requirements Healthcare background with passion for patient advocacy and support Strong communication and interpersonal skills Ability to build trust and rapport with diverse patient populations Self-motivated approach with ability to work independently and as part of a team Commitment to continuous learning and professional development Tertiary qualifications in counselling, psychology, or behavioural health disciplines are particularly welcome.
