• TYLER, TX, Salary Range: $47000.00 - $52000.00Salary, Job Shift: Day, The Center at Grande is looking for a Case Manager to join our team! What You’ll Do Track and record patient case information, progress, and evaluate recovery. Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance. Work with team members to ensure discharge planning goals and objectives are developed. Schedule appointments, and coordinate care What You’ll Need: Excellent verbal and written communication with professional phone etiquette.
