Schedule Sunday: 10:00 AM – 9:00 PM Monday: 3:00 PM – 9:00 PM Thursday: 3:00 PM – 9:00 PM Position Summary The Direct Support Professional (DSP) – Home and Community provides individualized support to people with disabilities, focusing on self-help, socialization, and independent living skills. Services are provided in the individual’s home and throughout their community, aligned with person-centered goals and outcomes.

Key Responsibilities

Support individuals in achieving goals through person-centered approaches. Maintain a balance between health and safety and individual independence. Assist with home and community-based activities that promote growth and engagement. Maintain professional relationships with families and team members. Attend team meetings and communicate regularly with supervisors.

Complete all required trainings and comply with incident reporting standards. Use a smart device for documentation and communication as needed. email jobs@achieva.info.

Qualifications
