Pharmacist, Ambulatory Pharmacy

Provides collaborative patient care with an interdisciplinary team, focusing on therapeutic drug monitoring including sedation, pain, and antimicrobial monitoring. Carries out IV to PO protocol when appropriate. Provides recommendations for appropriate stewardship of antimicrobials. Documents all interventions and activities appropriately. Serves as a drug information expert resource and provides information to patients and clinicians as needed. When staffing necessary, performs drug distribution services, patient monitoring, preparation of IV admixtures and sterile products, drug information, inventory control, checking work of technician job functions, maintains documentation competency, and provides input as needed.
