Key Responsibilities

Provide person-centered support to individuals with intellectual and developmental disabilities in their homes and community settings. Assist individuals with activities of daily living, including personal care, meal preparation, household tasks, and daily routines. Promote independence by teaching and reinforcing daily living, social, and community integration skills.

Support individuals in participating in recreational activities, appointments, shopping, and other community outings. Implement individualized service plans and behavioral support strategies as applicable. Assist with medication administration as authorized and trained. Monitor individuals' health, safety, and well-being and promptly report concerns.

Complete accurate and timely documentation of services and supports provided. Maintain positive communication with individuals, families, supervisors, and other members of the support team. Provide transportation as required for appointments and community activities.

Qualifications

High school diploma or GED preferred. Previous experience working with individuals with intellectual and developmental disabilities (IDD) preferred. Valid driver's license and acceptable three-year driving record required. Ability to successfully complete required training and certifications, including CPR/First Aid/AED, medication administration, and other applicable training.

Ability to provide hands-on personal care and support individuals with varying levels of independence. Strong communication, organizational, and interpersonal skills. Dependable, patient, compassionate, and committed to person-centered services. Ability to meet applicable background screening and employment requirements.
