At Stevens Treatment Programs, we support adolescents and young adults as they build the skills, relationships, and confidence needed for greater independence. Our Transitional Living Program works with youth on the practical and personal pieces of moving toward adulthood—from education and employment to housing, relationships, community connections, and long-term stability.
The Role As the TLP Case Manager , you’ll support adolescents and young adults as they prepare for greater independence and a successful transition into adulthood. This is an active, relationship-based role that combines individualized case management with hands-on work in the community. You’ll help young people identify goals, build practical independent-living skills, navigate education and employment, access community resources, and address needs related to housing, transportation, health care, and other areas of adult life.
You’ll help youth strengthen connections with family members and other important people in their lives and support individualized life-skills and permanency planning. You’ll serve as an important link between youth, families, DCF, schools, employers, community providers, and the treatment team—coordinating services, appointments, referrals, meetings, and other supports.
You’ll also complete required assessments and case management documentation and play an active role in treatment planning, transition and discharge planning, and aftercare. Schedule Monday, 9:00 AM – 5:00 PM Tuesday, 11:00 AM – 7:00 PM Wednesday, 9:00 AM – 5:00 PM Thursday, 12:00 PM – 8:00 PM Friday, 9:00 AM – 5:00 PM Payrate: $25.
00-$25.50 Who You Are You’re organized, dependable, and able to manage a lot of moving pieces without losing sight of the young person at the center of the work. You communicate well, build relationships naturally, and are comfortable working independently in the community as well as collaboratively with a multidisciplinary team.
Qualifications
High school diploma required; Bachelor’s degree preferred Case management experience required Excellent engagement, communication, and organizational skills Ability to work independently and responsibly in community settings Valid driver’s license and acceptable driving record Personal vehicle with current, valid vehicle registration and inspection Proof of automobile insurance with the following minimum coverage levels: $100,000 bodily injury per person $300,000 aggregate bodily injury $100,000 property damage, or $300,000 combined single limit Ability to complete required Residential Counselor and behavioral-support training, including physical intervention/restraint training when necessary Why Stevens?
