What You’ll Do Perform and document comprehensive initial evaluations to identify patients’ needs and establish individualized treatment plans. Develop and deliver therapeutic programs to restore function, improve mobility, relieve pain, and prevent physical decline. Monitor patient progress and adjust treatment plans to support the best possible outcomes.

Educate patients and families on exercises, safety, and strategies for maintaining independence. Collaborate with patients, families, and other healthcare professionals to ensure continuity of care. Follow infection control practices and universal precautions. Complete accurate documentation within company-established timelines.

Qualifications

Graduate of an accredited physical therapy program with successful completion of the state licensure examination. Current, valid Physical Therapist license. Current CPR certification. Valid driver’s license and auto insurance. One year of home health experience preferred. HomeCare HomeBase experience preferred. Physical Requirements Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
