How you’ll Contribute Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate.

Participates in discharges planning process. Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues Play a key role in the discharge planning process to support safe, successful transitions Why Join Us We believe that investing in our employees is the first step to providing excellent patient care.

What we are looking for

Certifications: AHA Basic Life Support (BLS) within 30 days of hire Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Experience: One year previous experience preferred Licenses: Active Wisconsin or compact state LPN license required.

If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.

Education
