You’ll assess patient needs, develop individualized treatment plans, and deliver therapy that improves mobility, reduces pain, builds strength, and restores function.

As a Physical Therapist, you will: Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans. Develop and implement individualized therapy programs to improve movement, restore physical function, relieve pain, and prevent further decline. Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.

Educate patients and caregivers on exercises, safe mobility, and strategies to support independence. Collaborate with patients, families, and healthcare team members to ensure continuity of care. Follow infection control practices and universal precautions during patient care. Complete accurate documentation within company-established timelines.

Support agency policies, procedures, and commitment to quality care.

Requirements

Graduation from an accredited physical therapy program and successful completion of the state licensure examination. Current, valid Physical Therapy license. One year of home health experience preferred. Current CPR certification. Valid driver’s license and auto insurance. HomeCare HomeBase experience preferred. Physical Demands Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

Ability to lift, carry, pull, or push up to 100 pounds.
