Your Role as a Licensed Practical Nurse (LPN)

As an LPN with Home Sweet Home, you will deliver hands-on skilled nursing care** to patients in their homes under the supervision of a Registered Nurse (RN).

Primary Responsibilities:**

Provide skilled, patient-centered nursing care based on individualized care plans

Accurately document all visits, assessments, and treatments using electronic health records (EHR)

Communicate effectively with patients, families, and the home health care team

Ensure compliance with Arizona home health regulations** and company policies

Help patients meet their health goals through compassionate and professional nursing

Qualifications for Home Health LVN Jobs

• Current Arizona Licensed Practical Nurse (LPN) license in good standing.
• Graduate of an accredited vocational nursing program**
• Minimum 1 year of nursing experience** (home health experience preferred)
• Physical ability to assist and move patients as needed
• Strong communication and documentation skills
• Proficiency with electronic charting and care coordination software**
• Valid driver’s license, reliable transportation, and ability to travel to patient homes in Central Phoenix and surrounding Maricopa areas**.
