Key Responsibilities

Patient Care Provide skilled nursing services as ordered by the physician and outlined in the plan of care Monitor patient condition, vital signs, and report changes to the supervising RN Perform treatments such as wound care, medication administration, and basic nursing procedures Assist patients with disease management and recovery Care Coordination Work under the direction of the RN to implement patient care plans Communicate effectively with physicians, nurses, and interdisciplinary team members Participate in case conferences and care coordination activities Support continuity of care across all services Documentation & Compliance Complete timely and accurate clinical documentation in the agency’s EHR system Ensure compliance with Medicare Conditions of Participation (Co.

Ps) and state regulations Follow agency policies and procedures regarding patient care and documentation Report incidents, changes in condition, and concerns to the RN/DON Patient & Family Education Educate patients and caregivers on medication management, treatments, and safety Reinforce teaching provided by the RN and other clinicians Promote patient independence and adherence to care plans Qualifications Active Licensed Vocational Nurse (LVN) license in Nevada Graduate of an accredited vocational nursing program Minimum 1 year of nursing experience (home health preferred) Basic knowledge of nursing principles and patient care practices Strong communication and organizational skills Ability to work independently under RN supervision Preferred Qualifications Previous home health experience Experience with wound care and chronic disease management Familiarity with home health EMR systems (e.
