Licensed Social Worker

• Conduct comprehensive psychosocial assessments of patients and families in their homes.

• Develop and implement individualized social work care plans in coordination with the interdisciplinary team.
• Provide counseling, crisis intervention, and emotional support to patients and caregivers.
• Assist patients and families with advanced care planning, community resources, and long-term care planning.
• Advocate for patients’ rights and help navigate healthcare systems and financial barriers.
• Collaborate with medical providers and clinical staff to support holistic care approaches.
• Document all services accurately and timely in the electronic medical record in compliance with agency and regulatory standards.
• Educate patients and families about available community services and eligibility criteria.
• Participate in case conferences, quality assurance, and staff development activities.
• Ensure adherence to all regulatory requirements including HIPAA and infection control guidelines.
