Social Worker - Outpatient Mental Health Services

This position performs a full scope of clinical medical social work services that include individual psychotherapy, biopsychosocial assessments, case management, coordination for skilled nursing facility placement, and collaboration with other hospital-wide case managers and/or social workers.
