Social Worker-Msw Rehab | Rehab Unit

The Social Worker completes psychosocial assessments and participates in treatment team meetings; provides individual and/or family conferences; screens patients on admission for transition of care; acts as a liaison with community resources/agencies: reports abuse and neglect to DHR; maintains a current list of community agencies/resources; assigns primary care physicians as needed; links patient with community resources i.

e. nursing home placement, home healthcare, hospice, transportation, as well as plans, organizes, coordinates, and implements comprehensive social work services to assigned patients. Incentives: $5,000 SIGN-ON BONUS Master's degree in social work from an accredited college or university and a current Alabama Social Workers license.

Minimum of six months acute care experience preferred. LGSW required, LCSW is a plus. Must possess Basic Life Support Certification by the American Heart Association or obtain with 30 days of hire.
