New grads are encouraged to apply! 🎓 Job Specifics: The Transitional Care Unit at Emplify Health by Bellin is a specialized 14-bed unit dedicated to providing comprehensive care for patients transitioning from an acute hospital setting to home or other care facilities. Our multidisciplinary team delivers personalized, patient-centered care aimed at optimizing recovery, enhancing functional independence, and ensuring a safe and seamless transition to the next level of care.

Location: 725 S. Webster Ave., Green Bay, WI 54301 FTE Status: Full and part-time positions available. Let us know what works for you! Work Schedule: A variety of shifts available: Day/Night, Day/PM, or PM/Night rotations. A weekend rotation included as well (every other or every third). Shifts/Hours: 8- or 12-hour shifts available.

Job Description: The Registered Nurse (RN) is responsible and accountable for providing quality professional nursing care to patients and their families and significant others adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures.

The responsibility includes providing a safe environment for patients, families, staff, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged. The Licensed Practical Nurse (LPN) is responsible for nursing care under the direct supervision of the Provider/Registered Nurse.

Records nursing care given and reports to appropriate person with changes in patient’s condition; consults with RN/Provider when uncertain about nursing or medical act; provides basic health instructions and reinforces teaching by other health team members.

Qualifications

Wisconsin RN license required for RN. Wisconsin LPN license required for LPN. New grads are encouraged to apply!
